Liability Release and Assumption of Risk

I understand and acknowledge that participation in trainings, practices, conditioning sessions, evaluations, events, and any related activities involves inherent risks, including but not limited to physical injury, illness, or other unforeseen harm.

On behalf of myself and my child, I voluntarily assume all risks associated with participation before, during, and after any Apex Batting Cages LLC activity. I hereby release, waive, and discharge Apex Batting Cages LLC, its coaches, staff, volunteers, representatives, agents, and affiliated organizations from any and all liability, claims, demands, or causes of action arising out of or relating to any injury, loss, or damage that may occur to my child, whether caused by negligence or otherwise, to the fullest extent permitted by law.

In the event of illness or injury, I authorize Apex Batting Cages coaches or designated representatives to obtain emergency medical care for my child as deemed necessary by qualified medical personnel. I understand that reasonable efforts will be made to contact me as soon as possible. I agree to assume full responsibility for all medical expenses and related costs incurred on behalf of my child.

I certify that my child is physically able to participate and that I have disclosed any relevant medical conditions. I have read this release, understand its terms, and sign it voluntarily with full knowledge of its significance.

“In the event of illness or injury to my child during activities, I authorize the coaching staff or designated representatives to obtain emergency medical treatment as deemed necessary by qualified medical personnel. I understand that reasonable efforts will be made to contact me immediately. I assume full responsibility for any medical expenses incurred on behalf of my child.”

“I grant permission to Apex Batting Cages LLC and it's affiliates to use my child’s name, image, likeness, and voice in photographs, videos, and other media for promotional, educational, and advertising purposes in print, digital, and social media platforms. I understand that no financial compensation will be provided, and that this release applies for the duration of my child’s participation in the program.”
